Transition chest note (C4) to falsetto (C5)?

Dear fellow members,
I'd hereby like to ask a question: is it possible to make a transition from singing a C4 in chest voice to singing a C5 in falsetto as Freddie Mercury does here in Exercises In Free Love? I attempted to recreate that part myself but I cannot imagine it being possible, although I am by no means a good singer. My guess is that it's simply a chest voice (C4) track and falsetto (C5) track being blended in post-production? What are your thoughts?
