What is the questions I send to KTVA for my first 1 hour session with him?

I lost my email that KTVA sent me for taking my first 1 hour session with Ken. I purchased the gold pack with 3 sessions, but I accidentally deleted the email with all the questions I have to answer to have my sessions with him. I'm about ready, but was waiting until I was fully prepared. Also, what is the email address I need to send my answers to?
