Cannot access any higher notes - help

in GENERAL SINGING - Ken Tamplin Vocal Academy Forum
As I sing the exercises my voice stops at D4 (and that's with some strain - no strain B4). No voice breaks or 'glip' seems I can't change "whatever" to get higher. What am I not doing or doing wrong? Also where in the HTSB course does it talk about moving to chest voice??

    Addition: went out and bought a torch so I could actually see the back of my throat. The uvula only moves up for about half it's length. Nothing I can do makes it move any higher
